No body can tell you for sure how many miles the engine will last.
Sense you are getting blue smoke on start up you will need guide work as well on the heads.

I've got 207k on the 350 in my 92 Silverado. As long as you keep the oil changed and aren't beating the hell out of the motor, it could last a while longer.

Problem is, I don't think you can get the heads of the engine in a van chassis without pulling the motor. To replace/repair the head, if you have to pull the engine, go ahead and refurbish the short block too.

On the heads, look at what the cost would be to freshen up BOTH heads (if one is having guide trouble, the other can't be far behind) and weigh that against the cost of a new set of heads that flow better and will increase performance, possibly even aluminum.

I think if you rebuild the heads that would kill the rings. Yes you can take the heads off of that van without pulling the engine, I done a head job on a 91 model. BUT, get you a carton of cigaretts and some good nerve pills before you start because it is a pain in the (be creative) to work on. Also the heads on these years are crap and prone to cracking. If it has ever been overheated they are most likely cracked. The ones on my van were but I used them any way. Good Luck.
